Athletic Team Registration

Physical Examination

Students registering for Athletics must have a current physical examination form on file before the student is authorized to participate in practice or tryouts. Physicals are valid for 1 year from the date of the exam.  You can upload a digital copy of the physical form during registration or you can send in a hard copy to the school nurse.
